All responses will remain confidential. No Information provided here will be shared with any outside parties. All entries must be received by September 1, 2008. The winner will be drawn by random computer selection from all valid entries received by the due date. We will require verification of your status as a currently licensed CRNA or Anesthesiologist before awarding the prize.

  

1. This vacation package will include lodging for a minimum of a 4-day / 3-night stay at the world-famous Atlantis Resort and Casino, Paradise Island, Bahamas. All other associated expenses not listed here as part of the prize, including, but not limited to additional transportation, food, beverages, activities, etc. are excluded and must be paid for by the winner.

2. Actual dates of travel are based upon availability and subject to approval by Nationwide Anesthesia Services, Inc. The winner needs to book this trip within 6 months of notification of winning.

3. There is no guarantee as to availability of travel dates and some restrictions may apply. Nationwide Anesthesia Services, Inc. determines these restrictions. Blackout dates will apply.

4. If a selected winner cannot be contacted, is ineligible or fails to claim a prize, an alternative winner will be selected.

5. Winner consents to the use of his/her name, photos and likeness for publicity and commercial purposes without further compensation unless prohibited by law.

6. Employees of Nationwide Anesthesia Services, Inc and their family and friends are ineligible for the contest.

7. Nationwide Anesthesia Services, Inc reserves the right to withdraw this contest without prior notice and to disqualify any winner.

8. Nationwide Anesthesia Services, Inc reserves the right to substitute prizes if circumstances so warrant.

9. Nationwide Anesthesia Services, Inc and its employees are not responsible for and shall not be liable for: (i) late, lost, delayed, damaged, misdirected, incomplete, illegible, or unintelligible entries; (ii) telephone, electronic, hardware, or software program, network, Internet, or computer malfunctions, failures, or difficulties; (iii) errors in transmission; (iv) any condition caused by events beyond the control of Nationwide Anesthesia Services, Inc that may cause the contest to be disrupted or corrupted; (v) any injuries, losses, or damages of any kind caused by a prize or resulting from acceptance, possession, or use of a prize, or from participation in the contest; or (vi) any printing or typographical errors in any materials associated with the contest.

10. Prizes are non-transferable, non-refundable and not exchangeable for cash or other prizes.

11. No purchase necessary to enter.

12. The odds of winning are dependent on the number of entries submitted.

13. The number of prizes awarded is at the discretion of Nationwide Anesthesia Services, Inc. A minimum of one vacation will be given away per calendar year.

14. To enter you must be a legal resident of the U.S. and a currently licensed Certified Registered Nurse Anesthetist or Anesthesiologist.

15. To enter you must complete all required survey questions and submit the survey before September 1, 2008.

16. All material, information, opinions and scores entered into the survey become property of Nationwide Anesthesia Services, Inc and can be used on the website NWAnesthesia.com, and/or in any other public or private media as Nationwide Anesthesia Services, Inc should deem desirable, with the exception specified in section 18 of this disclaimer.

17. All submissions of written and/or spoken materials and pictures become property of Nationwide Anesthesia Services, Inc.

18. Personal Information provided by contestant, such as name, address, phone and email addresses will not be sold or shared with any third party. This information is treated as confidential, and only used as outlined in section 5 of this disclaimer.

19. This trip must be used by the winner of the sweepstakes within one year of the date the prize is awarded.

20. The winner will be notified by the e-mail address or telephone number provided on the entry and will be announced on NWAnesthesia.com. Should the winners contact information change, they should notify Nationwide Anesthesia Services, Inc at: contest@nasinc.net. The winner must redeem and claim their prize within 30 days upon attempted notification by Nationwide Anesthesia Services, Inc. Nationwide Anesthesia Services, Inc is not responsible for unsuccessful prize notification due to changes in the entrants contact information.

21. If the prize is not redeemed within the 30 days, a 2nd drawing will be held 45 days after the initial drawing. The same rules above will apply to any subsequent drawings.
